The Chronic Disease Self-Management Program was developed by Stanford University, for seniors 60+, and facilitated by experienced, trained and certified volunteers. Learn how to manage pain, fatigue and frustrations; to build a healthful diet and exercise plan; to communicate with health providers and family members; and how to set and reach goals. This is a two-hour interactive weekly session, for six consecutive weeks, and attendees will receive a free book, Living a Healthy Life with Chronic Conditions. Family members and caregivers welcome. Sponsored by Meals on Wheels of Ocean County and Hackensack Meridian Health in partnership with the Ocean County Library. Funded in part by a grant from the Ocean County Board of Commissioners.
