If you have a child with intellectual and/or developmental disabilities, you may be anxious about their transition from PerformCare (New Jersey's Children's System of Care) to the New Jersey Division of Developmental Disabilities (New Jersey's Adult System of Care). During this workshop, Pat Llewellyn of the Center for 21+ will explain the process in clear, easy-to-understand language that will ease your concerns.
Pat will discuss the crucial step of applying for Medicaid, the four pathways to obtaining coverage, and steps to take to ensure that the coverage continues. He will describe the NJ DDD application process, documents that must be submitted for approval, and the variety of services that the agency provides.
Participants will receive handouts providing brief summaries of the NJ DDD, SSI, and Medicaid.
Pat Llewellyn has dedicated the last 20 years to supporting individuals with intellectual and developmental disabilities and the families who stand behind them. He began his career has a Direct Support Professional in a group home, later becoming a behaviorist, and went on to spend more than a decade with the New Jersey Division of Developmental Disabilities.
In the years that followed, Pat worked as a private consultant and advocate, helping families navigate complex systems, solve difficult programs, and secure the services their loved ones needed. He now coordinates adult services for the Center for 21+, continuing his mission of guiding families through every phase of adulthood.
