Join us for an informational program about the steps that occur when your disabled child ages out of the New Jersey Children's System of Care (PerformCare).
The New Jersey Division of Developmental Disabilities (DDD) oversees services for disabled adults. Monique Lewis Hawkins, Director of Equal Opportunity Support Services, will describe how to make the transition from one state agency to another as seemless as possible to ensure that your loved one continues to receive the services that they need.
The workshop will cover the following topics:
· How to find out whether a loved one is eligible for services
· The application process (including the differences between the long and short applications)
· The services that DDD offers
· What happens once an individual is notified that they are eligible for services
The workshop will be led by Monique Lewis Hawkins, Director of Equal Opportunity Support Services
